Follow-up rehabilitation and transfer of care
Follow-up rehabilitation (AR), also known as follow-up treatment (AHB), is one of the medical rehabilitation measures and is carried out following a stay in hospital. In specialist rehabilitation facilities, the aim is to improve your health and performance as comprehensively as possible through an all-day therapy concept.
The therapy usually lasts 3 weeks and is provided either as an outpatient or inpatient treatment. The difference is that the patient remains in their home environment during outpatient treatment and visits the outpatient rehabilitation center every day.
In the case of inpatient follow-up rehabilitation, the patient remains in a rehabilitation clinic for the duration of the therapy. AR must begin no later than 14 days after discharge from hospital. For medical reasons, follow-up rehabilitation can also be carried out up to 6 weeks after discharge.
Transfer to follow-up rehabilitation
If your doctor has registered an AR for you at our clinic, you will be contacted personally by the transfer management staff on the ward one day after the operation (Monday to Friday, usually between 14:00 and 16:00).
Please contact the transition management staff before you are admitted to hospital so that an appointment can be booked at a rehabilitation facility if possible. This applies in particular if you are to be transferred directly after your hospital stay.

The application procedure and the AR clinics differ depending on the cost bearer of the AR. The first step is therefore to clarify with you who the payer for your AR is.
As a rule, the German Pension Insurance (DRV) is responsible for persons capable of working. As there are various pension providers (e.g. DRV Bund in Berlin or DRV Bayern Süd in Munich), please let us know with which provider your pension account is held.
If you have full private health insurance, the DRV is also responsible if sufficient contributions have been paid.
If you are a civil servant, please let us know the relevant subsidy office and supplementary insurance.
If you are already a pensioner (old-age pension and unlimited reduced earning capacity pension), your health insurance provider is responsible for paying the costs.
Please note that many funding agencies only book certain rehabilitation facilities. Appointment bookings can therefore only be made at the funding provider’s contracted clinics. Some insurance providers register their insured persons in a rehabilitation clinic themselves after the application has been submitted. In this case, we only have limited influence on which rehabilitation clinic you are transferred to.
If an appointment can be made in advance at a rehabilitation clinic, this is always subject to cost approval.
Please have the following information ready when contacting the transition management:
- Pension provider with social security number
- Statutory health insurance with insurance number
- Private health insurance with policyholder number and tariff details
- If available: Aid office with reference number
Transfer of care
If you are in need of nursing care, we will help you to organize an outpatient nursing service or short-term care and clarify the requirements for an urgent nursing degree classification with you. If you already have a recognizable need for post-operative support before the operation, please contact us in advance during our telephone consultation hours.
