discharged directly home or transferred to follow-up treatment, depending on your clinical picture and treatment. If you require further care, we will take care of individual discharge preparation during your stay. This includes planning further medical and nursing care as well as organizing outpatient or inpatient rehabilitation measures or transfer to short-term or inpatient care facilities.  \t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-59329d9 elementor-widget elementor-widget-heading\" data-id=\"59329d9\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Discharge management<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-18b15cd elementor-widget elementor-widget-text-editor\" data-id=\"18b15cd\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>Special features for patients with statutory health insurance (GKV):<\/strong><\/p><p>The legislator has amended the German Social Code Book V (SGB V) to include new regulations on discharge management. From October 1, 2017, supplementary regulations apply to SHI patients (Section 39 (1a) sentence 9 SGB V). Upon admission to hospital, patients with statutory health insurance are informed about discharge management by means of a nationally standardized contract, which also provides for the transfer of patient data in the course of communication with external service providers or the health and long-term care insurance fund. Discharge management, which is regulated by law, is a further service for our patients, but is not mandatory. If you do not give your consent, you will continue to be provided with services as part of care management in accordance with Section 11 (4) SGB V for post-inpatient treatment. These can be used independently of participation in discharge management. As part of care management, the service providers must also guarantee appropriate follow-up care for the insured person.        <\/p><p>For our patients with statutory health insurance, however, the new regulation only changes what was already possible for our privately insured patients: namely that, depending on requirements, discharge management now also includes the prescription of medicines, dressings, remedies and aids, home nursing care or sociotherapy by hospital doctors who have completed specialist training in order to ensure seamless care for a period of up to seven days. Incapacity for work can also be determined for this period. These activities were previously reserved for doctors in private practice.
